Why does placement change how much it hurts?

Four things do almost all of the work. Every region above points back to whichever of these actually applies to it — this is the part with the “why,” not just the “how much.”

01Bone right under the skin

Where there’s no muscle or fat between skin and bone — shins, ribs, spine, kneecaps, ankles, the top of the foot — the needle’s vibration has nowhere to absorb into. You feel it as a sharper, more concentrated sting instead of a dull buzz.

02Nerve density

Hands, feet, the inner elbow, the armpit and the groin all carry a disproportionate number of nerve endings for their size — it’s the same reason they’re ticklish. More nerves reporting back means more signal, plain and simple.

03Thin, delicate skin

Skin thickness genuinely varies by location — the eyelids and the soles of the feet are the two extremes. Thinner skin over a sensitive area (wrists, inner arms, the sternum) means the needle is working closer to nerve endings with less of a buffer.

04Movement and friction

Ribs move every time you breathe. Joints bend mid-session. Skin over a moving structure keeps shifting slightly under the needle, which reads as more discomfort than the same depth of line on a flat, static spot like the outer thigh.

Does gender actually change this? Mostly no — bone proximity and nerve density are anatomy, not sex. A shin hurts about the same on anyone. The one place it genuinely differs is tissue type: breast tissue is more nerve-dense than the flat pec muscle it replaces in men, so the chest is a real exception, not a blanket rule for the whole body. We’ve called that out specifically where it applies, rather than doubling every entry on a difference that mostly isn’t there.

Front

Head / Scalp mainly bone + thin skin

Because the scalp isn’t flat, expect your artist to work in smaller sections than usual. Thick or textured hair can also genuinely mask outline crispness once it grows back in over the piece — worth discussing before committing to fine detail here.

Neck (front) mainly thin skin + nerve

If it’s your first tattoo anywhere near the throat, ask your artist to mark the exact line first. This is one of the few placements most studios want you fully rested and unhurried for, given how close the skin sits to major blood vessels.

Shoulder mainly bone

The skin here is loose and easy to stretch taut, which is part of why artists like it for bigger, flowing designs that need to wrap smoothly around the curve of the deltoid.

Chest mainly nerve + thin skin

For men: flat pec muscle over the ribs — tolerated well toward the outer chest, sharper right over the sternum in the center.

For women: breast tissue is nerve-dense and mostly lacks the muscle padding a man’s pec provides, so expect more sensitivity than the outer chest overall, especially closer to the center and underside. Worth a real conversation with your artist about placement and sizing before booking.

Ribs mainly bone + movement

Most people find the pain compounds a little as a longer rib session goes on, simply from the repeated motion of breathing. If you’re booking a large rib piece, plan for a short mid-session break.

Upper arm mainly bone

This is often the first placement recommended to a nervous first-timer for exactly this reason — a large, forgiving canvas with genuinely low pain across almost all of it.

Inner elbow mainly nerve + bone

Some artists avoid running a design directly through the crease itself even when the pain is otherwise acceptable, simply because the skin folds there constantly and can distort a tight line over the years.

Forearm mainly bone

Popularity aside, it’s also one of the easiest spots to hold visibly still for — which matters more for fine-line accuracy than most people expect going in.

Hand mainly bone + nerve

Beyond the session itself, hands are worth a real conversation about longevity — expect more touch-ups here over the years than almost anywhere else on the body, since they take more sun and washing than anywhere else you’d tattoo.

Stomach mainly nerve

Because this is an area that can change shape more than most over a lifetime (weight, pregnancy), many artists will talk through design choices — like avoiding a tight, literal outline right across the navel — before you commit.

Groin / bikini line mainly nerve + thin skin

Most studios that offer this placement at all will want a longer, unhurried consultation first — not only because of the pain, but because keeping clean lines on skin this mobile takes real technical skill.

Thigh (outer) mainly bone

The size and low pain together make this the default spot for a first large-scale piece — plenty of room, plenty of natural cushioning.

Shin mainly bone

Because there’s so little muscle to work with, artists often go a little slower here than on a fleshier spot — which can make the session feel longer even for the same amount of actual ink.

Knee mainly bone + movement

The joint bending mid-session is a real practical factor, not just discomfort — expect your artist to ask you to hold one position for longer stretches than on most other placements.

Ankle mainly bone + thin skin

Skin here tends to be thin enough that healing takes more care than average — sock and shoe friction is a real factor in the first couple of weeks, not just an afterthought.

Foot (top) mainly bone + nerve

Between the bone density and how much a foot moves and rubs against footwear while healing, this is one of the higher-maintenance placements for the first two weeks, pain aside.

Back

Upper back mainly bone

The size and flatness make it one of the best spots for a single large composition that needs a big, uninterrupted canvas.

Shoulder blade mainly bone

Designs that follow the natural curve of the scapula tend to read better than ones that fight against it — worth raising with your artist at the sketch stage, not mid-session.

Spine mainly bone + thin skin

Because there’s so little cushioning, most artists recommend keeping a spine piece to a single sitting where possible, rather than splitting one long line across multiple sessions.

Lower back mainly thin skin

Skin here moves more than the upper back does when you bend or sit — worth factoring into how intricate a design you go for.

Back of upper arm mainly bone

A common pairing with a front-of-arm piece, since both sides of the same arm can be tattooed as one continuous wraparound design.

Glutes mainly bone

Being one of the least painful spots on the entire body, it’s a genuinely practical choice for a large piece if you have a lower pain tolerance elsewhere.

Back of knee mainly nerve + movement

Because it’s a fold of skin rather than a flat surface, expect your artist to stretch it taut section by section rather than working it all at once.

Achilles / back of ankle mainly bone + thin skin

Small spot, but one that sees a lot of friction from shoes and socks during healing — pick footwear carefully for the first couple of weeks.
